Submitted By Tammy Sedore on Tuesday, April 2, 2019
Anyone wishing a passport to tryout for AA in another centre can contact Sean Whittaker [email protected]  We have not yet received these from OMHA but should soon have them.

Permission to skate for AAA should be directed to [email protected]

Permission to Skate forms for York Simcoe Express AAA tryouts

The Ontario Minor Hockey Association requires that every player trying out for a AAA team provide their AAA zone with a Permission to Skate form which they must obtain from their home centre prior to stepping on the ice.  In order to receive a PTS form from GMHA, a player must be a resident of Georgina.  Please note that a permission to skate form is not a player release nor is it an OHF waiver form.

The GMHA can only provide a PTS form for the YSE for players eligible to play with YSE.  We cannot provide a PTS form for any other AAA zone.  If a player is cut from the YSE they can request an OHF AAA Waiver from YSE to allow them to tryout with an adjoining AAA zone.

To speed up the process, please send an email to [email protected] with the following information, a PTS will be prepared for you.

Info Required:

Players Name:

Date of Birth:

Residence (full address):

Previous seasons team:

YSE Team player is trying out for:

PTS forms will be emailed only, their is no office pickup.

To obtain a JR PTS follow the above steps, a separate PTS is required for each Junior team.
