2013-2014 Coach Evaluations
It's that time of the year for Coach Evaluations.
This year GMHA is using Survey Monkey for this survey, and we hope that each parent can take time to complete the short survey.
The purpose of this evaluation is to assist GMHA in the evaluation of our Coaches and Coaching Staff.
The data collected in this questionnaire will be kept anonymous and confidential. The information gathered from the evaluations is used when considering coach applications, year-end awards and as reference to allegations made toward coaching staff, at which time there will be a follow-up to the evaluation submitted.
One survey will be accepted per player.
